This assignment offers the opportunity to work independently while providing one-on-one care to patients in their homes throughout the community. Ideal for an experienced Home Health PT who enjoys autonomy, flexibility, and building strong patient relationships. This contract is with CenterWell Home Health and requires prior Home Health experience with strong OASIS and documentation skills.
