Suncrest Hospice - - Responsibilities: Provide resident care under the direction of a Case Manager/DON; Follow resident assessments and care plans; Coordinate proper types of care with licensed care staff; Adhere to all nursing policies and safety guidelines; Collaborate daily with nurses, social workers, chaplains, volunteers, and STNA leadership
