The Norfolk Community Services Board (NCSB) provides treatment, support, and education to the community with a focus on mental illness, substance use addiction, and intellectual/developmental disabilities. Norfolk CSB provides services to persons in acute crisis as well as integrated, person-centered, and recovery-oriented programming for persons needing assistance and support stabilizing in the community.
Today, the NCSB provides treatment and rehabilitation services to over 6,000 Norfolk citizens yearly. Norfolk CSB seeks team members interested in providing skilled services in a public service environment. The NCSB is currently seeking a passionate and dedicated Substance Use Case Manager III to join our Mental Health and Substance Use Outpatient Department! In this role, you will provide direct case management and support services to individuals with substance use disorders, helping them access treatment, recovery resources, and community support systems. You will collaborate closely with clinical teams, ensure that services align with individualized treatment plans, and maintain documentation in compliance with Medicaid, NCSB, licensure, Human Rights, and other regulatory standards. Essential Functions

  • Provides direct serves to consumers at least monthly to monitor for stability and compliance with Individual Service Plan.
  • Assures that each consumer on the caseload has a formally developed treatment plan component that addresses the goals, strengths and skills of the consumer as well as any Identified deficits.
  • Identifies and assesses resources that meet the needs identified in the assessment process and assists consumers in procuring and maintaining resources and entitlements.
  • Refers consumers to the Norfolk CSB substance abuse programs for ongoing treatment services and make referrals to community agencies or other Norfolk CSB programs when appropriate.
  • Assures the maintenance of case management records on assigned caseload to include needs assessment, treatment, plan, applicable releases of information, monthly/quarterly progress review, current progress and all other documents/information that would support documentation of professional services.
  • Initiates and follows up with consumers on caseload that drop out of substance abuse services and provide follow-up services as necessary.
  • Attends monthly staff meetings with other clinical staff.
  • Informs supervisor of possible discharge from services and/or agency when consumers do not respond or comply with treatment planning or when consumers cannot be contacted or located.
  • Provides crisis counseling and intervention when necessary.
  • Arranges transportation and assists consumers with attending medical appointments, ASO appointments, Social Security appointments and other social agencies' appointments and provides other assistance as necessary.
  • Submits a daily activity sheet, weekly labor report and monthly schedule of planned activities.
  • Provide general case management services.
  • Liaison for referrals for detox and residential substance use treatment services.
  • Completes other special projects and duties as assigned.
Education/Experience
Required:
  • Bachelor's Degree in a human services field.
  • Two year's experience in a human services field related to the areas of assignment. Some assignments may also require specific experience working with various types of client populations.
Preferred:
  • Two years of substance use or co-occurring disorders experience.
Additional Information & Requirements
  • Valid driver's license.
  • Must be certified (or certifiable) in Basic First Aid and CPR.
Work Schedule:
  • Primary Hours, Monday-Friday 8:30am-5pm, 40 hours weekly

  • Retirement
If you are hired October 5, 2010, or after to a position with City of Norfolk Retirement System benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.

If you are hired January 1, 2022, or after to a position with Virginia Retirement System (VRS) benefits, you will be required to contribute five percent (5%) of your salary toward your retirement benefit. This will be a pre-tax payroll deduction.
  • The Tuition Assistance Program is established to encourage employees toward continued self-development and education. Permanent full-time and permanent part-time classified, unclassified permanent, full-time special projects, and constitutional employees who have completed six months continuous service will be eligible to apply. The applicant's school of enrollment must be an accredited institution.
